ABSTRACT

Introdução: os dados epidemiológicos mostram um aumento da população idosa vivendo com HIV/AIDS no Brasil. Esta doença impacta de diferentes formas a vida da pessoa constituindo um desafio para os serviços de saúde. Objetivo: verificar as alterações físicas, emocionais e sociais produzidas pelo HIV/AIDS na vida de idosos. Metodologia: estudo qualitativo com método história oral temática. Realizado no Serviço de Atendimento Especializado (SAE) em HIV/AIDS de uma cidade do Oeste Catarinense com a inclusão de três idosos, duas mulheres e um homem, com média de idade de 66 anos e diagnóstico do HIV/AIDS há mais de cinco anos. Foram realizados cinco encontros, dois de observação no domicílio e três para realização da entrevista em profundidade com validação de todas as etapas. A análise dos dados foi realizada por meio da análise de conteúdo temática. A pesquisa foi aprovada no comitê de ética. Resultados e discussão: os idosos apresentaram sintomas físicos decorrentes da imunossupressão sendo o principal deles o emagrecimento, que se constituiu como um sinal de alerta para o diagnóstico do HIV/AIDS. As repercussões emocionais foram o medo e a angústia, mais presentes no momento do diagnóstico e início dos tratamentos. As repercussões sociais estiveram atreladas ao isolamento social, saída de grupos e perda do papel no trabalho. Considerações finais: o HIV/AIDS repercute na vida dos idosos e produz diferentes alterações, quer sejam físicas, emocionais e/ou sociais. Nessa direção, incluir essas demandas no planejamento de ações para esse grupo populacional tornará a assistência à saúde mais resolutiva.(AU)


Introduction: epidemiological data show an increase in the elderly population living with HIV/AIDS in Brazil. This disease affects people's lives in different ways and constitutes a challenge for health services. Objective: To verify the physical, emotional, and social repercussions caused by HIV/AIDS in the lives of the elderly. Methodology: a qualitative study which used thematic oral history as a method. The research was carried out at the Specialized Care Service (SAE) on HIV/AIDS in a city in western Santa Catarina, which included three elderly diagnosed with HIV/AIDS for more than five years. Two meet-ings were held for approximation and three moments of in-depth interviews with validation in all stages. Data analysis had performed through thematic content analysis, according to Minayo (2014). Results and discussion: The elderly presented physical symptoms resulting from immunosuppression, the main one being weight loss, which constitutes a warning sign for the diagnosis of HIV/AIDS. The emotional repercussions were the fear and anguish most present at the time of diagnosis and the beginning of treatments. The social repercussions were linked to social isolation, leaving groups, and losing the social role of work, being partially reintroduced. Final considerations: HIV/AIDS affects the lives of the elderly, producing different changes, whether physical, emotional, and social, and health services and an interprofessional team need to ensure a broader view of this whole context to qualify assistance.(AU)

ABSTRACT

ABSTRACT Objectives: to analyze the concept of "early diagnosis of HIV/Aids infection" in light of Walker and Avant's conceptual analysis model. Methods: concept analysis study based on the framework proposed by Walker and Avant, instrumented by a scoping review conducted in April 2022, following the recommendations of the Joanna Briggs Institute and checklist Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. The search was made in eight data sources, obtaining sixteen articles. Results: the study found homosexual intercourses, early examination, anti-HIV antibodies, CD4 count, and sexually transmitted infection as the main attributes of the concept. As antecedents: information, risky behavior, unprotected sexual relations, prevention, and access to the service. As main consequences: antiretroviral treatment, seroconversion, transmission, and consultations. Final Considerations: the study approached the circumstantial situations of the theme, its attributes, antecedents, and consequences, qualifying the work process based on knowledge of nursing practice.


RESUMEN Objetivos: analizar concepto "diagnóstico precoz de la infección por VIH/SIDA" fundado en modelo de análisis conceptual de Walker y Avant. Métodos: estudio de análisis de concepto basado en referencial propuesto por Walker y Avant, instrumentalizado por revisión de ámbito realizada en abril de 2022, siguiendo las recomendaciones de Joanna Briggs Institute y checklist Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. Búsqueda hecha en ocho fuentes de datos, obteniendo 16 artículos. Resultados: verificado como principales atributos del concepto: Relaciones sexuales homosexuales, Examen precoz, Anticuerpos Anti-VIH, Recuento de CD4 e Infección sexualmente transmisible. Como antecedentes: Información, Conducta de riesgo, Relaciones sexuales sin protección, Prevención y Acceso al servicio. Como principales consecuentes: Tratamiento antirretroviral, Seroconversión, Transmisión y Consultas. Consideraciones Finales: el estudio acercó las situaciones circunstanciales de la temática, sus atributos, antecedentes y consecuentes, calificando el proceso de trabajo fundamentado en el conocimiento de la práctica de enfermería.

Resumo Objetivo: Analisar o itinerário terapêutico de idosos vivendo com HIV em assistência num município do Oeste Catarinense. Metodologia: Estudo qualitativo com método história oral temática. A população foi composta por idosos que vivem há mais de cinco anos com HIV/Aids, assistidos em um Serviço de Atendimento Especializado. Foram realizados dois encontros para aproximação e três momentos de entrevistas em profundidade abordando questões relacionadas as trajetórias assistenciais dos sujeitos e sua relação com a doença. A análise de dados foi realizada por meio da análise temática de conteúdo. Resultados e Discussão: Os idosos tiveram seu diagnóstico em fase tardia o que mostra a dificuldade dos serviços de saúde em identificar precocemente o HIV na pessoa idosa. O tratamento foi centralizado na medicação com antirretrovirais, de maneira setorializada em serviço especializado e com equipe constituída por profissional médico, enfermeiro e farmacêutico. Também foi evidenciado baixo acesso a recursos não farmacológicos, como assistência psicoterapêutica. Considerações finais: A trajetória assistencial precisa ser qualificada na direção de garantir maior acesso as redes de saúde e as equipes interprofissionais para uma atenção à saúde integral que realize o diagnóstico precoce com vistas a reduzir os riscos de complicações e ofereça um cuidado integral e humanizado, que extrapole o uso de medicamentos.


Abstract Objective: To analyze the therapeutic itinerary of elderly people living with HIV assisted in a municipality in Western Santa Catarina. Methodology: Qualitative study with thematic oral history method. The population was consisted of elderly people who have been living with HIV/Aids for more than five years, assisted in a Specialized Care Service. Two meetings were held for approximation and three moments of in-depth interviews. Data analysis was performed through thematic content analysis. Results and Discussion: The elderly had their diagnosis at a late stage, use antiretrovirals in their therapeutic journey, they have the specialized service as the central treatment space and greater contact with the medical professional, nurse and pharmacist. Final considerations: The therapeutic itineraries were built in a way that diagnoses happen late, with treatment focused on medication, in a sectorized way and without a diverse interprofessional team.

Abstract Rapid Diagnostic Tests (RDT) are useful to identify syphilis cases, particularly for hard-to-reach populations and if laboratory services are scarce. However, RDT performance may be suboptimal. We aimed to assess the sensitivity and specificity of a syphilis RDT using well-characterized blood donors' samples. We categorized samples from 811 blood donors into five groups: 1 - Samples with reactive Chemiluminescence (QML), FTA-Abs, and VDRL; 2 - Samples with reactive QML and FTA-Abs, and nonreactive VDRL; 3 - Samples with reactive QML, and nonreactive for other markers (false-positives); 4 - Controls with nonreactive QML; and 5 - Samples reactive for HIV, with nonreactive QML. Sensitivity was tested in groups 1 (overall and according to VDRL titers) and 2; specificity was tested in groups 3‒5. The RDT had high specificity, even in samples reactive for HIV. The sensitivity was high (91.9%) in samples with reactive VDRL but varied between 75.0%‒100% according to VDRL titers. The overall sensitivity was lower (81.3%) in samples with reactive FTA-Abs and nonreactive VDRL. The RDT is a useful tool to detect active syphilis but may be more limited for cases with very early or remote infection, or those with prior treatment. When higher sensitivity is needed, additional strategies including recurrent testing or laboratory-based tests may be required.

ABSTRACT Serodiagnosis of strongyloidiasis is usually performed by ELISA for the detection of IgG antibodies due to its high sensitivity and practicality, but its main limitation is a constant source of S. stercoralis antigens. The use of S. venezuelensis as a heterologous source of antigens has facilitated several published studies on the serodiagnosis and epidemiology of human strongyloidiasis. The main objective of this study was to evaluate the diagnostic accuracy of surface cuticle antigens of infective larvae of S. venezuelensis extracted with CTAB detergent (L3-CTAB) in comparison with soluble somatic extracts (L3-SSE) using a panel of sera from immunocompetent and immunocompromised individuals, at three different cut-offs. ROC curve analysis showed that L3-CTAB had an AUC of 0.9926. At the first cut-off value (OD 450 nm = 0.214), sensitivity and specificity were 100% and 90.11%, respectively, with a diagnostic accuracy of 0.93. At a second cut-off value (OD 450 nm = 0.286), sensitivity and specificity were 70% and 100%, respectively, with a diagnostic accuracy of 0.91. However, at an alternative third cut-off value (OD 450 nm = 0.589), sensitivity and specificity were 95% and 97.8%, respectively, with a diagnostic accuracy of 0.97. Using L3-CTAB as an antigenic source, the seropositivity rate in immunocompromised patients was 28.13% (9/32) whereas a seropositivity rate of 34.38% (11/32) was found when L3-SSE was used in ELISA. Therefore, the L3-CTAB is simple and practical to obtain and was found to be highly sensitive and specific.

Introdução: Com a emergência do SARS-CoV-2 foi disponibilizado uma grande quantidade de ferramentas de diagnóstico. Neste contexto, a falta de vacina, de tratamento e o grande número de casos graves e morte, possibilitou a aprovação emergencial de diversos testes, que ainda necessitam de estudos populacionais para seu registro definitivo. Objetivo: Realizar uma revisão de literatura para avaliar as metodologias de diagnóstico disponíveis no Brasil, de acordo com a realidade local de saúde, explorando o momento epidemiológico a complexidade do teste e a finalidade da sua aplicação. Metodologia: Trata-se de um estudo bibliográfico, descritivo do tipo revisão de literatura. Foram utilizadas as seguintes bases de dados científicos para buscas: PUBMED, MEDLINE, LILACS E COCHRANE LIBRARY, através de descritores selecionados na plataforma DECS. Resultados: O cenário de diversos ensaios, baseados em diferentes metodologias, como os testes baseados em RNA viral, em detecção de antígenos virais ou de anticorpos, associados ao conhecimento da história natural do vírus, possibilita uma análise crítica do melhor diagnóstico de acordo com a clínica do paciente, os epidemiológicos, o objetivo do diagnóstico e a acurácia do ensaio. Atualmente, há mudança no padrão imunológico da população e a descrição de tipos e subtipos de SARS-CoV-2 com mudanças gênicas, que podem levar a mudanças na acurácia diagnóstica ou a re-emergência em surtos de doença grave. Conclusão: Ainda é incerto o caminho evolutivo da história natural da Covid-19 e os ensaios diagnósticos estão em diferentes estágios de desenvolvimento, validação e produção e cada tipo de teste tem suas próprias vantagens e desvantagens distintas inerentes a plataforma tecnológica de origem e uma combinação de tipos de testes usados em momentos diferentes pode ser útil para a condução clínica dos pacientes e no controle da pandemia por SARS-CoV-2.


Introduction: With the emergence of SARS-CoV-2, a large number of diagnostic tools were made available. In this context, the lack of vaccine, treatment and the large number of severe cases and death, allowed the emergency approval of several tests, which still require population studies for their definitive registration. Objective: To carry out a literature review to evaluate the diagnostic methodologies available in Brazil, according to the local health reality, exploring the epidemiological moment, the complexity of the test and the purpose of its application. Methodology: This is a bibliographic, descriptive study of the literature review type. The following scientific databases were used for searches: PUBMED, MEDLINE, LILACS AND COCHRANE LIBRARY, through selected descriptors on the DECS platform. Results: The scenario of several tests, based on different methodologies, such as tests based on viral RNA, on detection of viral antigens or antibodies, associated with knowledge of the natural history of the virus, allows a critical analysis of the best diagnosis according to the patient's clinical, epidemiological, diagnostic objective and assay accuracy. Currently, there is a change in the immune pattern of the population and the description of types and subtypes of SARS-CoV-2 with genetic changes, which can lead to changes in diagnostic accuracy or the re-emergence in outbreaks of severe disease. Conclusion: The evolutionary path of the natural history of Covid-19 is still uncertain and diagnostic assays are at different stages of development, validation and production and each type of test has its own distinct advantages and disadvantages inherent in the technology platform of origin and a combination of types of tests used at different times can be useful for the clinical management of patients and in the control of the SARS-CoV-2 pandemic.


Introducción: Con la aparición del SARS-CoV-2, se dispuso de un gran número de herramientas diagnósticas. En este contexto, la falta de vacuna, tratamiento y el gran número de casos graves y muerte, permitieron la aprobación de urgencia de varias pruebas, que aún requieren estudios poblacionales para su registro definitivo. Objetivo: Realizar una revisión bibliográfica para evaluar las metodologías diagnósticas disponibles en Brasil, de acuerdo con la realidad sanitaria local, explorando el momento epidemiológico, la complejidad de la prueba y la finalidad de su aplicación. Metodología: Se trata de un estudio bibliográfico, descriptivo, del tipo revisión de literatura. Para las búsquedas se utilizaron las siguientes bases de datos científicas PUBMED, MEDLINE, LILACS Y COCHRANE LIBRARY, a través de descriptores seleccionados en la plataforma DECS. Resultados: El escenario de varias pruebas, basadas en diferentes metodologías, como pruebas basadas en el ARN viral, en la detección de antígenos virales o anticuerpos, asociado al conocimiento de la historia natural del virus, permite un análisis crítico del mejor diagnóstico de acuerdo con la clínica del paciente, epidemiológica, objetivo diagnóstico y precisión de la prueba. Actualmente, hay un cambio en el patrón inmunológico de la población y la descripción de tipos y subtipos de SARS-CoV-2 con cambios genéticos, que pueden conducir a cambios en la precisión diagnóstica o la reaparición en brotes de enfermedad grave. Conclusiones: El camino evolutivo de la historia natural del Covid-19 es aún incierto y los ensayos de diagnóstico se encuentran en diferentes etapas de desarrollo, validación y producción y cada tipo de prueba tiene sus propias ventajas y desventajas distintas inherentes a la plataforma tecnológica de origen y una combinación de tipos de pruebas utilizadas en diferentes momentos puede ser útil para el manejo clínico de los pacientes y en el control de la pandemia de SARS- CoV-2.

ABSTRACT Introduction: Congenital syphilis is a major public health problem, and early diagnosis and treatment are necessary to prevent it. Penicillin G benzathine is the treatment of choice in pregnant women; however, it may fail to prevent fetal infection, as in the present case. Case presentation: Male newborn, son of an HIV negative mother with gestational syphilis (venereal disease research laboratory (VDRL) 1:4 dilution, positive treponemal test) diagnosed at week 21 of gestation and treated with three doses of 2 400 000 IU of penicillin G benzathine. At delivery, the mother presented VDRL 1:1 dilution. The newborn was diagnosed with congenital syphilis due to VDRL 1:4 dilution, positive treponemal test, elevated aspartate aminotransferases, hypos-thenuria, proteinuria, hematuria, and leukocyturia that resolved after treatment with crystalline penicillin for 10 days. The molecular testing in blood showed a high treponemal load. The VDRL test at 3 months was non-reactive. Conclusions: Preventing congenital syphilis with the recommended treatment for gestational syphilis may fail. Moreover, diagnosing this condition in an asymptomatic newborn is difficult. Therefore, clinical and serological tests are recommended to confirm whether maternal treatment was effective in the fetus.


RESUMEN Introducción. La sífilis congénita es un importante problema de salud pública y para prevenirla es necesario diagnosticar y tratar la sífilis gestacional de forma temprana. En el presente caso la gestante recibió el tratamiento de elección (penicilina benzatínica), pero este no previno la infección fetal. Presentación del caso. Recién nacido masculino, hijo de una madre con serología negativa para el virus de la inmunodeficiencia humana y positiva para sífilis gestacional diagnosticada en la semana 21 (prueba VDRL con dilución 1:4 y prueba treponémica rápida positiva) y tratada con tres dosis de 2 400 000 UI de penicilina benzatínica. En el parto, la madre presentó VDRL con dilución 1:1 y el recién nacido fue diagnosticado con sífilis congénita por presentar VDRL con dilución 1:4, prueba treponémica rápida positiva, niveles de aspartato aminotransferasa elevados, hipostenuria, proteinuria, hematuria y leucocituria, condiciones que se resolvieron luego de recibir tratamiento con penicilina cristalina durante 10 días. El estudio molecular en sangre realizado al momento del nacimiento evidenció una alta presencia de Treponema pallidum. La prueba VDRL a los 3 meses fue no reactiva. Conclusiones. Prevenir la sífilis congénita con el tratamiento recomendado para sífilis gestacional puede fallar, además, diagnosticar sífilis congénita en un recién nacido asintomático es difícil, por lo cual se recomienda hacer un seguimiento clínico y serológico para confirmar si el tratamiento materno fue efectivo en el feto.

Objetivo: Analizar los cuidados de enfermería en pacientes portadores de VIH con diagnóstico de Bronco Neumonía. Método: Se realizó una revisión bibliográfica de documentos científicos basados en los cuidados de enfermería en pacientes portadores de VIH con diagnóstico de Bronco Neumonía. Resultados: Se seleccionaron 15 artículos científicos que van en el rango de publicación del 2020 - 2021 con aportaciones relevantes. Conclusión: La función del profesional de enfermería es atender tanto a la persona sana como enferma, en todo tipo de actividades que contribuyen a su salud o a recuperarla, se estima que del 5 al 20% son afectados, de esta manera es la etiología más frecuente en la población infectada por el Virus de Inmunodeficiencia Humana con mayor incidencia en la población masculina joven.


Objective: To analyze nursing care in HIV patients with a diagnosis of Bronchopneumonia. Methods: A literature review of scientific papers based on nursing care in HIV patients with a diagnosis of Bronchopneumonia was carried out. Results: 15 scientific articles with relevant contributions were selected within the publication range of 2020 - 2021. Conclusion: The role of the nursing professional is to care for both the healthy and sick person, in all types of activities that contribute to their health or to recover it, it is estimated that 5 to 20% are affected, in this way it is the most frequent etiology in the population infected by the Human Immunodeficiency Virus with higher incidence in the young male population.

ABSTRACT OBJECTIVE Recognize incident infection to better characterize the groups that fuel HIV epidemic. We propose a simple score to identify recent infections among newly diagnosed patients as a HIV surveillance tool. METHODS Newly diagnosed patients were defined as recent infections when a negative serological test in the previous year was available. Laboratory tests, such as the avidity index (Bio-Rad, according to the CEPHIA protocol), chemiluminescent intensity (CMIA, architect, Abbott), and the nucleotide ambiguity index of partial pol sequences were used as proxies of recency. A simple score based on clinical symptoms of acute retroviral syndrome during the previous year, CD4+ T cell count, and viral load at admission was tested to assess the predictive power, using receiver operating characteristic (ROC) curves, to identify recent cases of infection. RESULTS We evaluated 204 recently diagnosed patients who were admitted to the Ambulatório de Referência em Moléstias Infecciosas de Santo André (Santo André Reference Infectious Diseases Outpatient Clinic), in the metropolitan region of São Paulo, Brazil, recruited between 2011 and 2018. An HIV-negative test in the year prior to enrollment was documented in 37% of participants. The proportion of cases classified as recent infections (less than one year), according to the laboratory proxies were: 37% (67/181) for an avidity index < 40%, 22% (30/137) for a CMIA < 200, and 68% (124/181) for an ambiguity index < 0.5%. Using different combinations of recency definitions, our score showed an area under the ROC curve from 0.66 to 0.87 to predict recency. CONCLUSIONS Using data from patients' interviews and routine laboratory tests at admission, a simple score may provide information on HIV recency and thus, a proxy for HIV incidence to guide public policies. This simple for the Brazilian public health system and other low- and middle-income countries.

Introduction: Primary syphilis is classically represented by a hard chancre, but other rare forms such as Follmann's balanitis are occasionally described. Objective: To show an iconography of the clinical presentations of the classic hard chancre and Follmann's balanitis, in parallel with the serological results in the course of diagnosis and treatment. Methods: Descriptive case report of a patient of an outpatient clinic for sexually transmitted infections in a tertiary hospital in the city of São Paulo, SP, Brazil. Conclusion: Syphilis lesions acquire different clinical expressions according to the natural evolution of the disease. Recognizing these presentations, as well as knowing how to interpret the serological results, is essential for the diagnosis and adequate treatment of the infection.


Introdução: A sífilis primária é representada classicamente pelo cancro duro, porém outras formas raras, como a balanite de Follmann, são ocasionalmente descritas. Objetivo: Apresentar uma iconografia dos estágios evolutivos da lesão clássica de cancro duro e da balanite de Follmann em paralelismo com os resultados sorológicos no curso do diagnóstico e do tratamento. Métodos: Relato descritivo evolutivo de paciente atendido em ambulatório de atendimento de infecções sexualmente transmissíveis de hospital terciário da cidade de São Paulo (SP), Brasil. Conclusão: As lesões da sífilis adquirem expressões clínicas diversas conforme a evolução natural da doença. Reconhecer essas apresentações, bem como saber interpretar os resultados sorológicos, é fundamental para o diagnóstico e o tratamento adequado da infecção.

ABSTRACT

RESUMEN Objetivo a) estudiar la movilidad y el turismo sexual que realizan trabajadores de taxis de la ciudad de Taxco, Guerrero, México y b) analizar los datos de casos confirmados del Virus de la Inmunodeficiencia Humana (VIH) y Síndrome de Inmunodeficiencia Adquirida (SIDA) en el municipio de Taxco, del año 2006 al 2016. Materiales y Métodos El estudio fue de tipo cualitativo no probabilístico. Se diseñó un cuestionario de entrevista de preguntas abiertas, y se aplicó a 30 trabajadores de taxis en la ciudad de Taxco, Guerrero, México. También se entrevistó a cinco médicos especialistas (informantes clave anónimos). Resultados El 60% (18 casos) del grupo entrevistado manifestó haber contratado sexo-servidoras. Las ciudades a donde se acudió en mayor medida para dicha actividad fueron Jojutla (Estado de Morelos), Iguala y Taxco (Estado de Guerrero) y Ciudad de México. Se registró un total de 72 casos confirmados de VIH y sida en los 10 años considerados. Conclusiones La ciudad de Jojutla fue el lugar más visitado en cuestión de movilidad con motivo de contratar dicho sexoservicio. En lo anterior influyeron determinantemente tanto los bajos precios, como la corta edad de sexoservidoras que lo frecuentaban. Finalmente, las afirmaciones de especialistas en salud entrevistados confrontan los datos estadísticos oficiales sobre casos de VIH/sida.


ABSTRACT Objective (a) To study the mobility and sex tourism carried out by taxi workers in the city of Taxco, Guerrero, México. (b) To analyze the data of confirmed cases of the Human Immunodeficiency Virus (HIV) and Acquired Immunodeficiency Syndrome (AIDS) in the municipality of Taxco, from 2006 to 2016. Material and Methods The study was qualitative, non-probabilistic. An open-ended interview questionnaire was designed and applied to 30 taxi workers in the city. Five specialist physicians (anonymous key informants) were interviewed. Results The findings show how 60% (18 cases) of the interviewed group stated that they had hired sex workers. The cities that were most attended for such activity were Jojutla (State of Morelos), Iguala and Taxco (State of Guerrero), and México City. A total of 72 confirmed cases of HIV and AIDS were registered in the 10 years considered. Conclusions The city of Jojutla was the most visited place in terms of mobility for the purpose of hiring this sex service. Both the low prices, to the young age of the sex workers who frequented it had a decisive influence on the above. Finally, the statements of health specialists interviewed confront the official statistical data on HIV/AIDS cases.

Resumen | Introducción. La sífilis gestacional se considera de interés en salud pública por las repercusiones que tiene en la madre y el hijo. Pese a tener protocolos para su notificación, diagnóstico y manejo, en Colombia se siguen evidenciando limitaciones en su control. Objetivo. Describir las características sociodemográficas, clínicas y de distribución espacial de las pacientes con sífilis gestacional en Cali, Colombia, en el 2018. Materiales y métodos. Se hizo un estudio transversal con 427 casos de sífilis gestacional reportados al Sistema de Vigilancia en Salud Pública (Sivigila). Para el procesamiento estadístico, se utilizó el programa R, versión 3.5.3. Las variables cualitativas se presentan como proporciones y, las cuantitativas, mediante medidas de tendencia central y dispersión, Para la distribución espacial, se usó el programa Qgis 3.0. Resultados. La razón de sífilis gestacional fue de 17 casos por 1.000 vivos (incluidos los mortinatos). El 57,1 % de las pacientes pertenecía al régimen subsidiado de salud y el 16,6 % no estaba asegurado. El 90,4 % de los casos se diagnosticó durante el embarazo; el 47,2% recibió tres dosis de penicilina y el 57,6 % de los contactos recibió tratamiento. Conclusiones. La tasa de sífilis gestacional en Cali en el 2018 fue superior a la nacional y la enfermedad se presentó con mayor frecuencia en mujeres gestantes en condición de vulnerabilidad socioeconómica, lo que coincidió con la distribución espacial en general. Se evidenció la falta de oportunidad en la detección temprana y el manejo de la infección tanto en las pacientes como en sus contactos, lo cual dificulta el control de la enfermedad y refleja la inadecuada aplicación de la ruta integral de atención en salud materno-perinatal.


Abstract | Introduction: Gestational syphilis is considered an event of public health interest given its impact on mother and child. In Colombia, despite having specific protocols for its notification, diagnosis, and management, there are still limitations in its control. Objectives: To describe the sociodemographic and clinical characteristics, as well as the spatial distribution of gestational syphilis in Cali, Colombia, in 2018. Materials and methods: We conducted a cross-sectional study of 427 gestational syphilis cases reported to the Colombian national epidemiological surveillance system (Sivigila). For the statistical processing, we used the R program, version 3.5.3. We expressed qualitative variables as proportions and quantitative ones through central tendency and dispersion measures, and to establish the spatial distribution we used the Qgis program, version 3.0. Results: The prevalence of gestational syphilis was 17 cases per 1,000 live births (including stillbirths); 57.1% of patients belonged to the subsidized healthcare system and 16.6% had no health insurance; 90.4% of cases were diagnosed during pregnancy; 47.2% of the pregnant women received three doses of penicillin, yet only 57.6% of contacts were treated. Conclusions: The prevalence of gestational syphilis in Cali during 2018 exceeded the national rate with a higher frequency among women in socioeconomic vulnerability conditions consistent with the general spatial distribution. There was a lack of opportunity in the early detection and management of the infection both among pregnant women and their contacts, which hinders the control of the disease and reflects the inadequate application of the comprehensive maternal and perinatal health care route guidelines.

Resumen Un cuadro de meningitis aséptica, en el contexto de un paciente inmunosuprimido con diagnóstico de infección por Virus de Inmunodeficiencia Humana (VIH), debe llevar a pensar en múltiples diagnósticos diferenciales. Entre estos, se incluye el virus varicela zóster (VVZ) como uno de los principales agentes causales de meningitis a líquido cefalorraquídeo claro. Su reactivación da lugar a múltiples manifestaciones neurológicas potencialmente mortales en las que se consideraba al rash vesicular, o exantema pápulo/vesículo/ costroso como un signo fundamental para su diagnóstico. No obstante, las lesiones cutáneas están ausentes en más de un tercio de los pacientes con compromiso del sistema nervioso central. A continuación, se presenta el caso de un paciente con infección por VIH que presenta cefalea más fiebre, con hallazgos en líquido cefalorraquídeo de pleocitosis neutrofílica y una prueba molecular confirmatoria para virus varicela zóster, en ausencia de rash vesicular previo que guiara hacia este diagnóstico. Abstract The clinical presentation of aseptic meningitis in the context of an immunosuppressed patient with a diagnosis of Human Immunodeficiency Virus (HIV) infection, should lead us to consider multiple differential diagnoses. Among these, the Varicella Zoster Virus (VZV) has been found as one of the main causative agents of clear cerebrospinal fluid meningitis. Its reactivation gives rise to multiple life-threatening neurological manifestations in which vesicular rash, or papule / vesicular / crusted rash was considered a fundamental sign for its diagnosis. However, skin lesions are absent in more than a third of patients with central nervous system involvement. Herein, we report a case of an HIV-infected patient with headache, fever and neutrophilic pleocytosis with FilmArray that confirms Varicella Zoster virus infection in an immunocompromised patient in the absence of vesicular rash. ABSTRACT Background: National rates of HIV, gonorrhoeae and syphilis have increased in Chile in recent years, but it is not known if syphilis among pregnant women (PW) is also escalating. Aim: To explore syphilis rates among PW in a southern Chilean region and to describe clinical features of mothers and offspring. Patients and Methods: We selected PW with positive VDRL or T. pallidum micro-he-magglutination (MHA-TP) reactive tests, referred to a high-risk obstetrical pathology center between 2011 and 2019. Clinical information of mothers and their offspring was obtained from their medical records. Results: Syphilis rates among PW increased from 0.4 to 7.2 per 1,000 live births in the reference center from 2013 to 2019, along with a national and regional increase in both sexes. Twenty-nine cases of PW with syphilis were identified with a median age of 28 years (interquartile range 23-32). Seventeen percent had a history of drug abuse, and 14% had previous sexually transmitted diseases. In seven cases (24%), the diagnosis occurred at delivery. Three percent of patients had a primary syphilis, 14% a secondary syphilis, 24% an early latent syphilis, and 59% a latent syphilis. All cases except one case were treated with benzathine penicillin G and the remaining with erythromycin due to allergy suspicion. Penicillin desensitization was required in two cases (7%). Treatment timing was inadequate in 10 cases (34.5%). Of 19 patients with available serological follow-up, 10 (53%) showed a significant decrease in VDRL titers (≥ 2 dilutions) and two cases had re-infection. Two patients (7%) had an abortion, two had congenital syphilis (7%), and six had premature births (21%), totaling 35% of adverse-pregnancy outcomes. Conclusions: Resurgence of syphilis among pregnant women is happening in southern Chile and is associated to adverse-pregnancy outcomes.


Antecedentes: Las tasas nacionales de VIH, gonorrea y sífilis han aumentado en Chile en los últimos años, pero no se sabe si la sífilis en mujeres embarazadas (ME) también está aumentando. Objetivo: Explorar las tasas de sífilis en ME en una región del sur de Chile y describir las características clínicas de las madres y los hijos. Pacientes y Métodos: Se seleccionaron ME con un VDRL o test de microhemaglutinación para T. pallidum positivos, referidas aun centro de alto riesgo obstétrico entre 2011 y 2019. La información clínica de las madres y sus hijos fue obtenida de fichas clínicas. Resultados: Las tasas de sífilis en ME aumentaron de 0,4 a 7,2 por 1000 nacidos vivos en el hospital de referencia entre 2013 y 2019, en paralelo a un aumento nacional y regional en ambos sexos. Se identificaron 29 casos de mujeres embarazadas con sífilis con mediana de edad 28 años, rango intercuartílico 23-32. Diecisiete por ciento tenía historia de abuso de drogas y 14% tenía una enfermedad de transmisión sexual previa. En siete casos, el diagnóstico ocurrió en el momento del parto (24,1%). Los estadios abarcaron sífilis primaria (3%), sífilis secundaria (14%), sífilis latente temprana (24%) y sífilis latente (59%). Todos, menos un caso, fueron tratados con penicilina G benzatina y el caso restante con eritromicina por sospecha de alergia a la penicilina. Se requirieron protocolos de desensibilización a penicilina en dos casos (7%). El momento del tratamiento fue inadecuado en 10 casos (35%). De 19 pacientes con seguimiento serológico disponible, 10 (53%) mostraron una disminución significativa en los títulos de VDRL (≥ 2 diluciones) y dos casos tuvieron reinfección. Dos pacientes (7%) evolucionaron con aborto, dos con sífilis congénita (7%, una asociada a terapia con macrólidos) y seis con parto prematuro o bajo peso al nacer (21%), totalizando 35% de resultados adversos durante el embarazo. Conclusiones: El resurgimiento de la sífilis entre las mujeres embarazadas está ocurriendo en el sur de Chile y está asociado a resultados adversos del embarazo.

Objetivo: Analisar o conhecimento de mulheres que realizaram consultas de pré-natal em relação à sífilis e as orientações recebidas acerca da prevenção de sífilis gestacional. Métodos: Pesquisa qualitativa e descritiva, desenvolvida com oito gestantes, em uma unidade de Atenção Primária à Saúde (APS), de um município de Fronteira Oeste, Rio Grande do Sul, Brasil, por meio da técnica de entrevista semiestruturada, no período de setembro a outubro de 2019. Os achados foram interpretados por meio da análise temática, emergindo duas categorias temáticas: Conhecimento sobre a sífilis e Orientações sobre a prevenção da sífilis na gestação. Resultados: As gestantes investigadas demonstraram conhecimento restrito sobre sífilis e sífilis gestacional. Relataram que as orientações no pré-natal são superficiais. Disseram que a transmissão da sífilis ocorre por via sexual e demonstraram surpresa quanto às complicações da doença para o bebê, evidenciando o desconhecimento sobre a sífilis congênita. Citaram o preservativo como método de prevenção, porém relataram não utilizar quando o parceiro é fixo. Demonstraram conhecimento restrito sobre a interpretação dos testes rápidos, não mencionando a realização do exame não treponêmico como método diagnóstico e confirmatório da doença. Conclusão: A lacuna identificada pelo conhecimento limitado das gestantes investigadas sobre a sífilis e a prevenção da sífilis gestacional pode ser suprida por meio da realização de atividades de educação em saúde, tendo o enfermeiro como agente promotor.


Objective: To analyze the knowledge of women who attended prenatal consultations concerning syphilis and the guidelines received about the prevention of gestational syphilis. Methods: It is qualitative and descriptive research, developed with eight pregnant women, in a unit of Primary Health Care (PHC), in a municipality of Fronteira Oeste, Rio Grande do Sul, Brazil, using the semi-structured interview technique from September to October 2019. The findings were interpreted using thematic analysis, with two thematic categories emerging: Knowledge about syphilis and Guidelines on the prevention of syphilis in pregnancy. Results: The investigated pregnant women demonstrated restricted knowledge about syphilis and gestational syphilis. They reported that the guidelines for prenatal care are superficial. They said that the transmission of syphilis happens through sex and showed surprise about the complications of the disease for the baby, showing the lack of knowledge about congenital syphilis. They mentioned the condom as a prevention method but reported not using it when the partner is fixed. They demonstrated limited knowledge about the interpretation of rapid tests, not to mention the non-treponemal exam as a diagnostic and confirmatory method of the disease. Conclusion: The gap identified by the limited knowledge of pregnant women investigated about syphilis, and the prevention of gestational syphilis can be filled by carrying out health education activities, with the nurse as a promoter.


Objetivo: Analizar el conocimiento de mujeres que tuvieron el prenatal sobre la sífilis y las orientaciones recibidas sobre la prevención de la sífilis gestacional. Métodos: Investigación cualitativa y descriptiva desarrollada con ocho embarazadas de una unidad de Atención Primaria de Salud (APS) de un municipio de la Frontera Oeste, Río Grande de Sur, Brasil a través de la técnica de entrevista semiestructurada en el periodo entre septiembre y octubre de 2019. Se ha interpretado los hallazgos a través del análisis temático del cual se ha identificado dos categorías temáticas a continuación: Conocimiento de la sífilis y Orientaciones sobre la prevención de la sífilis durante el embarazo. Resultados: Las embarazadas investigadas han demostrado poco conocimiento de la sífilis y de la sífilis gestacional. Ellas han relatado que las orientaciones del prenatal son superficiales, que la transmisión de la sífilis se da por la vía sexual y han demostrado sorpresa sobre las complicaciones de la enfermedad para el bebé lo que evidencia la falta de conocimiento de la sífilis congénita. Ellas han citado el condón como el método de prevención, sin embargo, relataron no usarlo cuando tiene más tiempo con su compañero. Las participantes han demostrado poco conocimiento de la interpretación de las pruebas rápidas y no han mencionado la prueba no treponémica como el método diagnóstico y confirmatorio de la enfermedad. Conclusión: Se pude arreglar la laguna del conocimiento limitado de las embarazadas investigadas sobre la sífilis y la prevención de la sífilis gestacional a través de actividades de educación en salud con el enfermero como el agente promotor.

Objetivo: Avaliar a percepção da pessoa vivendo com o vírus da imunodeficiência humana sobre a ocorrência de atitudes discriminatórias em atendimento odontológico. Métodos: Trata-se de um estudo qualitativo, realizado com pessoas vivendo com o vírus da imunodeficiência humana e em tratamento em um centro de referência, na cidade de Governador Valadares, Minas Gerais, Brasil, no período de janeiro a julho de 2019. Foram incluídos 25 participantes no estudo, tendo sido realizadas entrevistas semiestruturadas. Os dados coletados foram submetidos à análise de conteúdo temática e, a partir daí, as categorias que emergiram incluíram o receio da informação do diagnóstico ao profissional de saúde e a recusa de atendimento por profissionais da saúde. Resultados: Entre os entrevistados, houve percepção de confiança para informação de sua condição sorológica ao profissional, embora também tenha ocorrido omissão dela. Foi percebida certa insegurança relacionada ao cirurgião-dentista na realização do tratamento odontológico, incluindo atitudes discriminatórias no consultório e até recusa de atendimento. Conclusão: Entre as pessoas vivendo com o vírus da imunodeficiência humana investigadas no presente estudo, houve percepção de condutas discriminatórias e dificuldades para conseguirem atendimento odontológico, além do receio de informar seu diagnóstico ao cirurgião-dentista, omitindo-o.


Objective: To assess the perception people living with the human immunodeficiency virus have in relation to the occurrence of discriminatory attitudes during dental care. Methods: This is a qualitative study conducted with people living with the human immunodeficiency virus undergoing treatment in a reference center in the city of Governador Valadares, Minas Gerais, Brazil, from January to July 2019. The study included 25 participants, and semi-structured interviews were carried out. The collected data were subjected to Thematic Content Analysis and the categories that emerged included fear of disclosing the diagnosis to the health care professional and health care professionals' refusal to provide care. Results: Among the interviewees, there was a perception of trust in disclosing serological status to the professional, but there were also some who hid it. Certain insecurity was noted in dentists while delivering dental treatment, and there were some discriminatory attitudes in the office and even refusal to deliver care. Conclusion: There was a perception of discriminatory behavior and difficulties in obtaining dental care among the people living with the human immunodeficiency virus analyzed in the present study. In addition, there was fear of disclosing the diagnosis to the dentist, and some hid it.


Objetivo: Evaluar la percepción de la persona que vive con el virus de la inmunodeficiencia humana sobre la ocurrencia de actitudes de discriminación durante la atención odontológica. Métodos: Se trata de un estudio cualitativo realizado con personas que viven con el virus de la inmunodeficiencia humana y en tratamiento en un centro de referencia de la ciudad de Governador Valadares, Minas Gerais, Brasil, en el periodo entre enero y julio de 2019. Se ha incluido 25 participantes en el estudio utilizándose de entrevistas semiestructuradas. Se ha realizado el análisis de contenido temático de los datos recogidos de los cuales emergieron las categorías que han incluido el miedo de la información del diagnóstico para el profesional sanitario y el rechazo de la atención de parte de los profesionales sanitarios. Resultados: Entre los entrevistados, hubo la percepción de confianza para dar la información de su condición serológica al profesional aunque también haya ocurrido la omisión de información. Se ha percibido cierta inseguridad relacionada al cirujano-odontólogo para la realización del tratamiento lo que incluye las actitudes de discriminación en el consultorio hasta el rechazo de la atención. Conclusión: Entre las personas que viven con el virus de la inmunodeficiencia humana investigadas en el estudio, hubo la percepción de conductas de discriminación y dificultades para conseguir la atención odontológica además del miedo para informar su diagnóstico al cirujano-odontólogo, omitiéndolo.

Objective:To systematically evaluate the efficacy of benzathine penicillin combined with ceftriaxone on the negative conversion rate of the rapid plasma reagin circle card test/toluidine red unheated serum test (RPR/TRUST) in the treatment of early syphilis.Methods:According to the search strategy, online databases (PubMed, Web of science, Embase, Cochrane Library, CNKI, Wanfang and VIP) were searched for case-control studies on benzathine penicillin combined with ceftriaxone in the treatment of early syphilis. The Newcastle Ottawa scale (NOS) was used to evaluate the quality of the included articles, and the RevMan5.3 software to analyze the negative conversion rate of RPR/TRUST.Results:A total of 14 case-control studies were included, including 1 160 syphilis patients (585 in the combination treatment group and 575 in the benzathine penicillin alone group) . According to the meta-analysis, the negative conversion rate of serum RPR/TRUST was significantly higher in the combination treatment group than in the benzathine penicillin alone group ( OR=3.70, 95% CI[2.71, 5.06], P<0.001) . Subgroup analysis based on the follow-up duration, the negative conversion rate of serum RPR/TRUST was also significantly higher in the combination treatment group than in the benzathine penicillin alone group after 3-month ( OR=3.68, 95% CI [2.26, 5.98], P<0.001) , 6-month ( OR=3.11, 95% CI[2.26, 4.27], P<0.001) and 12-month treatment ( OR=4.35, 95% CI[2.81, 6.73], P<0.001) . Conclusion:Compared with benzathine penicillin, benzathine penicillin combined with ceftriaxone can more effectively promote serum RPR/TRUST to turn negative in the treatment of early syphilis.

@#Goat Warble Fly Infestation (GWFI) is also known as subcutaneous myiasis caused by Przhevalskiana silenus (Diptera: Oestridae). It is widely distributed in tropical and sub-tropical areas of the world. In goats, WFI is usually detected through conventional procedure which underestimated the infestation. The current study was designed to determine the serodiagonsis of GWFI (through IDEXX Hypodermosis serum antibody test) and also aimed to investigate its seroepizootiological profile in Pothwar region, Pakistan from 2013-14. The results showed that average seropositivity (ELISA kit) of GWFI was 18.5% whereas, it was 11% by using conventional procedure (Palpation method) depicting a significant difference (p<0.05). Higher seropositivity (30.8%) was observed in Jhelum district as compared to e Attock district (6%). The L1 larvae were found in September, while nodules start appearing in October to December and last until the end of February. The month wise peaks of optical density (OD) was higher in December which gradually decrease along with the end of winter season. The prevalence of GWFI revealed no significant difference among three host breeds (Jattal, Beetal and Tedy). According to the results, high infestation rate (28%) was observed in young animals of age group < 1 year as compared to old animals (> 2 years). Topographically, hilly areas (33%) provide favourable climatic conditions for the propagating of larval stages. Sex difference showed no significant difference. The seroprevalence varied significantly with respect to age, month, districts and topographical location. The current study proved that serologic diagnosis (commercial ELISA kit) as more sensitive and accurate for timely diagnosis of GWFI than traditional method. The information on the epizootiology of P. silenus in goats of Pothwar region would help in devising effective control strategies.
